Forms
STAC-202 Forms [PDF]
The STAC-202 form indicates that a student has been designated as homeless, and specifies the name of the school the student will be attending. It can also serve as a reimbursement form for the local school to use to request full tuition reimbursement for educating the temporarily housed student.
STAC-4 Forms [PDF]
STAC-4 forms are used for general state aid on a per-pupil expenditure basis. After the local school district has completed and sent in the STAC-202 to the STAC Office, the STAC Office will send the district a partially completed STAC-4 form for the district to complete. The district must then complete the STAC-4 forms and return the forms to the STAC Office.
