New York City Residency Questionnaire [TIF]

In order to identify students in temporary housing situations, this Residency Questionnaire should be given to each New York City public school student at the start of the school year, and the completed form should be added to each student's education file. Throughout the school year, each student who enrolls in a New York City public school or changes his/her address should also be given this form.

Sample Letter Requesting Yellow Bus Service for a General Education Student in New York City [Word doc]

All students in temporary housing who are in grades K-6 should get busing if there is an appropriate busing route. To request busing, families can speak with the Department of Education worker (also known as the Family Worker or Family Assistant) in the shelter, or speak with the borough's Students in Temporary Housing (STH) Content Expert.

Housing Assistance for Families with ACS Cases [Word doc]
