STAC Forms
The System to Track and Account for Children (STAC) forms are used by NYSED exclusively for reimbursement purposes. Completed STAC forms can also help school districts meet their data reporting requirements, because the STAC forms track the number of homeless students and living situations of these students. STAC-202 Forms [PDF]
STAC-202 forms indicate that a student has been designated as homeless, and specifies the name of the school the student will be attending. It can also serve as a reimbursement form for the local school to use to request full tuition reimbursement for educating the temporarily housed student. STAC-4 Forms [PDF]
STAC-4 forms are used for general state aid on a per-pupil expenditure basis. After the local school district has completed and sent in the STAC-202 to the STAC Office, the STAC Office will send the district a partially completed STAC-4 form for the district to complete. The district must then complete the STAC-4 forms and return the forms to the STAC Office. Find answers to commonly asked questions about STAC-4 forms.
